Copy the file from the 'Watch_Dogs Niko' folder in the following path
Rockstar Games-Grand Theft Auto IV-pc-models-cdimages
replace the file 'playerped.rpf'
All files in download:
Watch_Dogs Niko - playerped.rpf
Original Niko - playerpedNIKO.rpf
README.txt

Everything looks awesome, unfortunately there is a minor glitch with the hat and the back of the head. Niko's real hair starts poking out, at least in my game. Still, besides the hat, the clothing and bandana looks awesome!
